Housing Stabilization Services
Housing Stabilization Services is a new Medical Assistance benefit in Minnesota to help people with disabilities, and seniors who are at risk of homelessness find and keep housing. Housing stabilization services are state plan (not waiver) Home and Community-Based Services (HCBS).
HSS is composed of three different services: Housing Consultation, Housing Transition and Sustaining
- Housing Consultation – service to develop a person-centered plan for people without MA case management services. This service supports an individual in building a housing focused person-centered plan. Housing Consultation services also support individuals in making informed choice of a housing transition or sustaining provider, in addition, consultation services assist individuals in connecting to resources that are non-housing related but are identified in an individual’s person-centered plan.After thats completed you will be referred to Us or another Housing transition/sustaining provider.
- Housing Transition – service that supports a person to find housing.
- Housing Sustaining – service that supports a person to maintain housing.
What We Do!!
- Contact the appropriate county agency.
- Initiate the housing process with potential relocation venues.
- Look for good affordable apartments for you.
- Make arrangements for tours to prospective relocation sites.
- Help you with the housing application process even if you have poor rental history.
- Assist you in contacting entities with movers, household items, furniture etc.
- Make arrangements for your transportation on your moving day.
- Connect you with other community resources and services after we move you into your new home.
To get started, you can meet with a Housing consultant to complete a housing focused person-centered plan. Or have your worker add it to your Coordinated Services Support Plan (CSSP) This plan will help us in providing you the support that you need to find and keep your housing.
